I understand that the information submitted by the Student Athlete is expected to be complete and accurate to the best of their knowledge, and that any falsification of information may result in the Student Athlete being declared ineligible for consideration.
I have read and understand the eligibility criteria for the JHSABC Scholarship Program, and I am aware of the corresponding deadlines for the application and all its parts, including essay, and 2 Student Evaluations. I understand it is THE STUDENT ATHLETE’S responsibility, NOT THE PARENT/GUARDIAN, to ensure all components are completed and submitted PRIOR TO the stated deadlines.
Further, I hereby authorize the review of all aspects of the Student Athlete’s application for the sole purpose of consideration of the JHSABC Scholarship Program, including but not limited to any necessary follow up communications with the JHSABC Executive Board, JHSABC Scholarship Committee, JHS Athletic Program, Teachers, Staff and/or any other individual(s) identified on the application.
I authorize the JHSABC to use my Student Athlete’s Name and Photo in any communications announcing scholarship award recipients, if applicable.
