Scholarship – Student Evaluation Form (2026)

The Jordan High School Athletic Booster Club (JHSABC) is proud to announce the 2025-2026 Scholarship Program. Ten (10) scholarships in the amount of $1000 will be awarded to eligible graduating senior athletes at Jordan High School.

As part of the application, the student athlete must receive two (2) Student Evaluations. A student athlete has requested you as an evaluator. The Student Athlete’s name and KISDID is contained in the email message that you received with the link for this form, and should be prepopulated in the form below.

Note: You may get multiple emails on behalf of multiple applicants. Please complete a separate form for each applicant, using the respective email that you received for each student athlete.

Please submit your online evaluation for this applicant by 11:59PM on Monday, March 31, 2026.
